A MAN who died following a police chase on the M4 had been reported missing on the day of the fatal crash, a watchdog has said.

The Independent Police Complaints Commission (IPCC) has begun an independent investigation after 35-year-old Andrew Gwynne died following a police pursuit on the M4 motorway in Cardiff on Saturday (5 August).

The collision happened near the Coryton interchange at junction 32 eastbound at around 6pm, when a red Ford Focus collided with a stationary Landrover on the hard shoulder.

The driver of the car, Andrew Gwynne, from Gloucestershire and formerly Chepstow, had been reported missing after concerns for his welfare were raised by his family earlier that day.

Initial information shows Mr Gwynne was seen driving on the M4 and attempts were made to stop his car including the deployment of a stinger.

The Landrover driver was taken to hospital but is not believed to have been seriously injured. Mr Gwynne was pronounced dead at the scene.

IPCC investigators attended the scene on Saturday evening and a local police station for the post incident procedure.

Initial accounts have been obtained from the police officers involved, who are being treated as witnesses. A significant number of members of the public who witnessed the events have been identified.

IPCC Operations Manager Mel Palmer said: “This was a tragic event and my thoughts are with Mr Gwynne’s family and friends and all those affected.

“We have now met with his family to explain our role and will continue to keep them updated as our investigation progresses.”
